Upon Admission

  • Where can a PhilHealth Member get the form to be accomplished? 

 The patient will be given the CF1 form to be accomplished by the PhilHealth Member and Employer.
A list of required documents that need to be submitted upon discharge will also be given.

Note: CF2 forms are given to the nurses’ station to be filled out by the Attending Physician.

During your stay

What are the Important Requirements for PhilHealth Benefits?

  • CF1 – Claim Form 1: Accomplished and originally signed by member and employer.
  • CF2 – Claim Form 2: Accomplished and originally signed by Attending Physician, Surgeon, Anaesthesiologist and patient or next of kin if patient is unable to sign.

What is the additional requirement for an Employed Member?

  • Copy of updated Member Data Record (MDR) or PhilHealth ID
  • For Dependent: Copy of updated MDR with list of dependents

What are the additional requirements for an Individual Paying Member or Voluntary Member?

  • Copy of updated Member Data Record (MDR) or PhilHealth ID
  • Clear copy of MI-5 or PhilHealth Official Receipt (latest, should be 9-12 months prior to admission)
  • For Dependent: Copy of updated MDR with list of dependents

What is the additional requirement for a Non-Paying Member or Lifetime Member?

  • Copy of Non-Paying Member or Lifetime Member ID (front and back)
  • For Dependent: Copy of updated MDR with list of dependents

What is the additional requirement for an Indigent Member?

  • Copy of valid Family Health Card or PhilHealth Para Sa Masa ID (front and back) with validity period
  • For Dependent: Copy of updated MDR with list of dependents

What is the additional requirement for an Overseas Worker?

  • Copy of updated MDR with Payment Information
  • For Dependent: Copy of updated MDR with list of dependents

Upon Discharged

What should the Member do upon Discharge?

  • Submit all completed PhilHealth documents and required documents upon discharge at the PhilHealth section (Ground Floor, Main Lobby).

PhilHealth Case Rate Packages

  • PHIC recently released a list of diagnosis and procedures with fixed amount coverage. If the patient’s diagnosis is any of the following, please ensure that the patient’s Attending Physician is PHIC-accredited and the required test results are attached to your claim form.
