PATIENT GUIDE

Admission

 We aim to make you feel as comfortable as possible as soon as you arrive at Notre Dame de Chartres Hospital. Upon entering the Admitting Section, the Admission Clerk will ask for the following:

  1. Physician’s Admission Order
  2. Health Care Insurance card or document
  3. Accredited HMO-issued member ID
  4. Letter of Authorization from your HMO/company (to validate if you are entitled for HMO/company hospitalization benefits).

Admitting Hours

 Admitting hours are 24/7. 

Admission Form

 Please fill up the Admission Form completely and accurately with the required information. This form includes your consent to medical or surgical intervention, acceptance of financial responsibility for services/charges not covered by your insurance, HMO or other sources. This is a very important procedure and we appreciate your cooperation. 

Admission Kit & Patient ID Bracelet

 The hospital provides each admitted patient with an admission kit containing a digital thermometer, medicine cup, calibrated drinking cup, spoon and fork, face towel, soap and tissue paper. Patient ID bracelet is also provided with your name, age and gender.

IMPORTANT: Please ALWAYS wear the Patient ID Bracelet during your whole hospital confinement. DO NOT remove your bracelet until after you are discharged from Notre Dame de Chartres Hospital.

Health Maintenance Organization (HMO) & Corporate Accounts

 Patients of companies, who are accredited with the hospital, shall present a Letter of Authorization (LOA) issued by the HMO/Company. Otherwise, the bill will be the personal account of the patient. 

PhilHealth

 Kindly inform the Admitting Clerk upon Admission if you are a PhilHealth member. PhilHealth forms are available at the Business Office-Philhealth Counter. Please submit your completed papers before discharge for you to be able to take advantage of your benefits. 

Visiting Hours

 Visiting hours for Non-COVID wards are from  11:00 AM – 1:00PM and 6:00 PM – 8:00 PM.  For ICU are from 11:00AM - 12:00NN and 6:00PM -7:00PM. Only immediate family members (patient's parents, siblings, spouse and children) are allowed to visit. Two visitors per patient are allowed at a time in order to  maintain adequate physical distancing or to effect other infection control measures.

For health reasons, children below seven (7) years old are not allowed in the hospital.

Dietary & Food Service

Registered dieticians prepare the meals of the patient. They also prepare special diets for patients following the prescription of attending physicians. Meals are served according to the following schedule:

Breakfast: 6:00 AM – 8:00 AM
Lunch: 11:00 AM – 1:00 PM
Dinner: 5:00 PM – 7:00 PM

Delivery of Food from External Sources

For security reasons, delivery personnel from external sources are NOT allowed to bring food to the patient’s room. Watchers of patients are requested to pick up their deliveries at the hospital lobby. 

Waste Management

The hospital follows a strict policy on waste management. Please dispose your waste in appropriate trash bins for Bio-degradable, Non-biodegradable and Infectious. Guidelines for proper waste disposal are given upon admission. 

No Styrofoam Policy

Upon entrance of clients to the vicinity, they are informed and oriented of the policy that styropore and plastic are not allowed in any area of the hospital. 

No Smoking Policy

 Smoking is strictly prohibited within the building and external property line of Notre Dame de Chartres Hospital. 

Pharmacy

Notre Dame de Chartres Hospital operates on a CLOSED DRUG POLICY, meaning all medications for inpatient has to be taken from the Hospital Pharmacy. This allows our pharmacists to properly monitor individual patient medications. Our Pharmacy is open 24 hours daily to serve you. 

Check Out & Discharge

 Your doctor’s order authorizing your discharge should be written on your chart. Should you decide to leave the hospital when such discharge is not ordered or against doctor’s advice, you should sign the “Discharge Against Medical Advice Form” and may check out after settlement of your account at the Business Office-Billing Section.

Secure your clearance slip from the Nurse’s Station after you settle your account at the Business Office-Billing Section.

The hospital has a 4:00 PM cut-off for discharged patients to check out of their room.
